The Care Plan
Help @ Home Senior Care offers caregiving services for a variety of needs and home/family situations. We strive to assess and understand the client's unique situation and, in very little time, recommend a care plan to best meet their individual needs. This way the client can receive the care he or she deserves as soon as possible.
The Initial Consultation
During the initial inquiry, we begin considering the specific needs of our clients and ask questions to form a basic needs assessment. Then we set up an in-home visit with the client to gather information, get acquainted with each other and their daily activities. The Custom Care Plan
After our initial meeting, we'll develop a customized care plan for your loved one, based on the information collected in our basic needs assessment, including detailed recommendations on the kind of services that would be most beneficial. We then review the care plan in detail and modify it as you see fit. Both the client and the caregiver sign, date and receive a copy of the plan.
Care Supervision
Every Help @ Home caregiver goes through an extensive interview process, including reference, criminal background and driving records checks. Once hired, caregivers are bonded, insured and trained to ensure that they're able to deliver the care set out in your loved one's care plan.
Help @ Home team members stay in touch with caregivers and clients to confirm things are going well. Team members also make periodic visits to the homes of those who are in our care to be sure caregivers are following the specifics of the care plan. We strive to maintain effective communication with everyone involved throughout the caregiving process.
